Procedure to Purchase BEM Form F / F1
- By a registered PE (Civil & Structural).
- Purchase maximum 10 copies at any one time.
- BEM/IEM/ACEM to record name and PE No. of each purchaser. PE stamp with signature or authorized letter with PE stamp with signature is required to be furnished to make a purchase.
- Each set of Form F/FI consisting of 4 sheets will be sold at RM30/-.

Conditions for Use
- Form F/FI is not transferable to another PSP for use
- Purchased Forms are not refundable
- If any unused form is discovered to be missing, it is the duty of the PSP to inform BEM in writing as soon as reasonable upon discovery.
